Scheduling: Select option 1 to schedule an office visit, test, or hospital admission.
Prescription Refills: Call desired office and select the prompt to speak to a nurse or medical assistant.
Leave your name, number, medication name, dosage, and how many times a day you take the medication. Allow 24 -48 hours to complete the request. You will be asked to come to the office to pick up your scripts. We encourage you to have refills completed at time of visit as you will be charged for refills requested between visits. Payment is expected at time of pick up.

Test Results: Call desired office and select the prompt to speak to a nurse or medical assistant.
All calls received before 2 pm will be returned between the hours of 3-5pm. Test results will be mailed to your home within 2 weeks. This allows the physician time to review and make recommendations. Patients will be called with the results of any abnormal testing, or to schedule further tests, or to schedule a follow up visit.
Medical Questions & Concerns
Each physician has a nurse and medical assistant to takes calls from their patients. Call desired office and select the prompt to speak to a nurse or medical assistant. Calls received by 2 pm will be returned that day.

Medical Records: Press option 4
To obtain medical records or for completion of any forms. We will assist in the completion of one form per year at no charge. Additional forms will be charged at a minimum of $10 per form. Please allow 2-4 weeks for forms to be completed.

Please direct any billing/insurance issues or questions to our billing service at 1-866-284-3845. We will be happy to assist you with any other concerns that are not addressed by the billing service by pressing option 6
