Office Procedures

Your physician has ordered testing that has been scheduled at our office. Please take the time to review the general instructions for the test you have been scheduled. If you have further questions about the study, please contact our office at 937-454-9527 ext. 3020 to reach our Special Procedures Department.

If you need to speak to the scheduler about your appointment time, please dial extension 3004. We require a minimum of 24 hours notice (Monday through Thursday testing) if you need to cancel or reschedule. If you are scheduled for Monday, the cancellation must be received by noon on Friday to avoid possible charges for supplies or medications that are non returnable and are ordered specifically for each patient. We appreciate your cooperation and understanding of this request. Please verify insurance with our scheduler, as many of these tests require precertification by your insurance company.
